“Access Form”
Project Domain / Category
Web Application
Abstract/Introduction
The objective of this project is to design and develop AccessForm, a web-based survey builder application that places web accessibility at its core. Unlike conventional tools such as Google Forms or SurveyMonkey, AccessForm is specifically designed to ensure that surveys are inclusive, usable, and accessible to all individuals, including those with visual, hearing, motor, or cognitive impairments. The project highlights strict compliance with the Web Content Accessibility Guidelines (WCAG 2.1). Students are expected to actively consult, interpret, and apply recognized accessibility standards and best practices throughout every stage of the design and development process.
A fully accessible web-based survey builder application, ensuring equal participation and usability for all users. Build a survey builder web application with accessibility-first design. Ensure all user interactions support keyboard navigation, screen readers, and high-contrast modes. Provide alternative input/output options (voice, SMS, captions, alt text). Deliver an analytics dashboard with accessible data visualization. Apply recommendations from official accessibility guides (e.g., WCAG, ARIA practices).
Functional Requirements:
User Roles
Admin Panel: Manage users, monitor surveys, and oversee accessibility compliance.
Form Creator Panel: Build surveys, customize accessibility settings, and distribute forms.
Respondent Panel: Complete surveys via web, mobile, voice, or SMS.

Deliverables
Final Report: Covering problem analysis, design, implementation, and evaluation.
User Manual/Guide: Instructions for form creators and respondents.
Testing Report: Usability testing with accessibility validation (WCAG compliance checks).
GitHub Repository: Complete source code with documentation.
Live Demo or Recorded Presentation: Demonstrating accessibility features in action. Students must ensure that accessibility is prioritized in every development stage. Teams must consult accessibility guides (WCAG, WAI-ARIA, best practices) when building the survey application.
